== What is this? ==

If the umbilical cord — the tube that carries blood and oxygen from the placenta to the baby — slips out of the birth canal before the baby's head, this is called cord prolapse. If an arm or leg of the baby appears at the birth canal opening before the head, this is also an emergency. In both cases, the cord or the baby's position is blocking or cutting off the flow of blood and oxygen to the baby.

Every minute without oxygen can cause permanent brain damage in the baby. This is the most urgent obstetric emergency in the MAMA ALERT deck. The only life-saving treatment is an emergency Caesarean section at a hospital with a functioning operating theatre. Nothing that can be done at home will fix this — only a hospital can.

== What is this? ==

Passing little or no urine for 12 hours or more means the kidneys are under severe stress. In a pregnant woman, this can happen because of very severe vomiting and dehydration, severe pre-eclampsia affecting the kidneys, or a serious body infection (sepsis). The lips and skin feel very dry, the eyes look sunken, and the woman may feel dizzy or too weak to stand.

This is not something that can be managed with oral fluids at home. She needs fluids given directly into a vein (IV drip) at the PHC or CHC. The sooner she gets there, the less damage occurs. Do not wait to see if passing urine improves on its own — it will not.

When a pregnant woman suddenly cannot stand or walk — even with someone's help — she is in a serious medical emergency. She may feel the room spinning, be too weak to lift herself, or be slipping in and out of consciousness. This level of weakness means the body is failing. Severe blood loss, severe dehydration, eclampsia, or another life-threatening condition may be happening.

Do not try to walk her to a vehicle or sit her upright on her own. She must be transported lying flat on a surface. Call 108 immediately. If she is conscious and can swallow, give small sips of ORS while waiting — but nothing by mouth if she is not fully awake. Every minute counts.

When a woman has had a baby who was born still, or who died within the first four weeks of life, the current pregnancy needs extra careful monitoring. A previous baby loss can have many causes — some of which can happen again if they are not identified and managed early. This does not mean this pregnancy will have the same outcome — but it means that more care is needed, starting from the very first day.

Register at the CHC or District Hospital as early as possible. Tell the doctor or ANM about the previous baby loss at every single check-up. Attend all ANC visits without missing any. If any new danger sign appears during this pregnancy, go to the facility the same day — do not wait.

When the inner eyelids and the fingernails look completely white or almost colourless, and the woman cannot stand without support, her blood count has fallen to a dangerously low level. This is severe anaemia. The body does not have enough red blood cells to carry oxygen to the muscles and organs. She may feel she is about to faint, or her head spins when she tries to stand.

This level of anaemia is a medical emergency. The woman may need a blood transfusion — a treatment that is only available at the CHC or District Hospital. Iron tablets will not work fast enough at this stage. She needs emergency hospital care right now.

If a pregnant woman suddenly collapses and cannot be woken, or cannot respond to her name or to touch, she has lost consciousness. This is a medical emergency. The cause may be severe fits (eclampsia), a very heavy internal bleed, dangerously low blood count, or a sudden severe drop in blood pressure. In every case, she must reach the CHC or District Hospital within minutes.

Do not try to wake her by shaking her hard. Do not give anything by mouth — no water, no food, no medicine. One adult must stay with her at all times. Phone 108 immediately. Another adult must ride in the ambulance with her — she must never travel alone.

== What is this? ==

A baby that has not moved at all for 12 hours or more is in a serious situation. A healthy baby moves throughout the day and night. A long silence — especially after the baby was previously active — can mean the baby is in serious distress and is not getting enough oxygen. This is very different from the quiet spells that last an hour or two when the baby is sleeping.

Do not wait until morning. Do not wait to see if movement returns. Go to the PHC or CHC immediately — day or night. The doctor will listen to the baby's heartbeat using a machine (cardiotocograph or Doppler). If the heartbeat is abnormal, the woman will need emergency care at the District Hospital.

Fits (convulsions) during pregnancy — where the body shakes uncontrollably and the woman loses consciousness — are a life-threatening emergency called eclampsia. This is caused by very high blood pressure that has reached the point where it affects the brain and nervous system. Without treatment within minutes, fits can cause permanent brain damage or death.

A fit can happen suddenly, even in a woman who seemed well before. It can happen before, during, or after delivery. == What is this? ==

A burning or stinging pain when passing urine, or needing to pass urine very often, is a sign of a urinary tract infection (UTI). UTIs are very common in pregnancy because body changes make it easier for bacteria to enter the bladder and kidneys. If not treated with the correct antibiotic, a UTI can spread to the kidneys and cause a serious infection (sepsis) that can trigger premature labour.

Do not try to treat a UTI on your own with leftover tablets, herbal remedies, or by drinking more water alone. The ANM or doctor will test the urine and give the correct antibiotic for pregnancy. Take every tablet in the full course — do not stop when the burning eases. Drink at least 8 glasses of water a day to support recovery.

When two or more babies are growing in the womb, the mother's body carries far more weight and works much harder than in a single pregnancy. She is more likely to feel very tired, short of breath, and uncomfortable. The babies may grow more slowly and are more likely to be born early — before they are fully ready.

When a baby was delivered by Caesarean section (C-section) in a previous pregnancy, a scar is left on the uterus. In the current pregnancy, this scar can sometimes tear open — especially if labour starts without the woman reaching a hospital first. This tearing is called uterine rupture and is a life-threatening emergency for both the mother and the baby.

This does not mean a normal vaginal delivery is impossible — but it means the delivery must happen in a hospital that can perform a C-section and provide a blood transfusion if needed. Feeling slightly breathless when climbing stairs or walking quickly in late pregnancy is common and usually not a problem. But if a woman cannot take a full deep breath even while sitting still and at rest, feels her chest is tight, or feels like she cannot get enough air, this is a danger sign. The body should always be able to breathe comfortably when at rest.

Difficulty breathing at rest can be caused by severe anaemia (not enough red blood cells to carry oxygen), a heart or lung condition made worse by pregnancy, or severe pre-eclampsia. It can also be a side effect of the baby pressing on the lungs in late pregnancy — but even then, the breathing should not cause distress at rest. High blood pressure in pregnancy — also called hypertension — means the reading is 140/90 mmHg or above on two readings taken 15 minutes apart. When the whole body swells at once — face, hands, and feet together — this is different from the ordinary ankle swelling of late pregnancy. When the eyelids puff up, the fingers become thick and tight, and the feet are swollen all at the same time, the body is holding far too much fluid. This type of swelling does not go away after a night of rest. It may build quickly over a day or two, or appear suddenly first thing in the morning. Pull down your lower eyelid gently and look at the inside surface. In a healthy person this is a deep, bright pink. If it looks pale pink, white, or almost colourless, it is a sign that the blood is low in iron. This condition is called anaemia. Also check the fingernails — pale, yellowish, or spoon-shaped nails point to the same problem.

Anaemia in pregnancy means the blood cannot carry enough oxygen to the mother's muscles and organs, or to the growing baby. == What is this? ==

A healthy baby moves regularly inside the womb from around the fifth month of pregnancy onward. You may feel kicks, rolls, or flutters many times each day. Babies also have quiet periods when they sleep — this is normal. But during the time the baby is usually active, it should move at least 10 times in two hours.

If the baby seems quiet, find a peaceful moment, lie down on your left side, and count every movement — every kick, roll, and flutter. Try drinking something cold or sweet first, as this often wakes the baby. If you still count fewer than 10 movements in two hours, this is a sign to go to the PHC the same day.

Mild swelling of the feet and ankles is common in late pregnancy, especially in the evening or after standing for a long time. The growing baby puts pressure on the blood vessels and the legs hold extra fluid. When this swelling goes away after a night of rest with legs raised, it is usually not dangerous.

Swelling that does not go down overnight, or that spreads to the face and hands, is a different and more serious sign. It can mean blood pressure is rising or there is a fluid problem that needs medical attention. A simple way to check: pull down your lower eyelid — if the inside looks white instead of deep pink, tell your ASHA the same day.

A headache that keeps coming back is not just simple tiredness. It means the head feels heavy, tight, or throbbing even after sleep and rest. It may be there most of the day, or it may return every day. This kind of headache can be a warning sign of high blood pressure in pregnancy, called pre-eclampsia. High blood pressure can harm the mother’s brain, kidneys, and other organs, and it can also affect the baby’s growth. That is why it is important to notice a headache that does not go away and take action early.[1]

Sometimes this headache gets worse over a few days. It can be felt more in the second and third months of pregnancy and later in pregnancy too. The woman may feel dizzy, tired, or feel pressure behind the eyes. If the headache comes with blurred vision, swelling of the feet or face, or pain in the upper belly, it becomes more dangerous. Early care can prevent serious problems like fits, stroke, or loss of the baby.[1]
